Hi there,
I am a Home Share provider and recently received a Health profile form in the mail and I am aware it is a silly question but do I fill out the Health information of the person in my care or that of my own?

This is a question that has come up from a few people so it can’t be that silly. The form is to be filled out for the person(s) you support.
