What is Transition Planning?

Transition planning is about preparing for your future.  It is specifically about preparing for moving from one phase of your life to another.  This could mean a change from adolescence to adulthood, adulthood to retirement, or a change in living situations.

Preparing for these changes is important but can sometimes be a bit confusing.  We’d like to offer you some links to information that may make those transitions and planning for them a little easier.
